Your attorney and financial institutions prepare the controlling instruments. Feline Transitions confirms what it has actually accepted and keeps verification with your private Lifetime Care record.
Two structures
A pet trust and a charitable contribution are not the same arrangement.
Individual animal carePet trust
A trustee controls property for one or more identified animals under the trust instrument. The trust should name an enforcer and explain care, payment, monitoring, successor appointments, and the final remainder.

Collective program supportLifetime Care contribution
A gift to Feline Transitions supports the shared program unless the organization expressly accepts a different restriction in writing. It does not create a participant-owned balance.

Funding instruments
Use the institution’s required form and keep proof of acceptance.
A will does not replace an insurance, retirement, POD, or TOD beneficiary designation. Each instrument should be coordinated with the Lifetime Care agreement and reviewed by the participant’s professional advisers.
01Pet trust or trust amendment
Creates a legally administered fund for identified animals. Your attorney should name the trustee, enforcer, caregiver process, permitted expenses, successors, and remainder recipient.
02Will or codicil
May fund a testamentary pet trust, make a charitable gift, and give the personal representative practical transfer directions.
03Life insurance
Use the carrier’s beneficiary form and obtain confirmation that the designation was accepted.
04Retirement account
Use the plan administrator’s form and obtain legal and tax advice, including any required spousal consent.
05POD or TOD designation
Use the financial institution’s form, identify the beneficiary precisely, and retain institutional confirmation.
06Direction letter
Give trustees and personal representatives practical contacts and transfer instructions. A direction letter does not amend a controlling legal instrument.
07Restricted Gift Agreement
Required when a donor proposes an enforceable restriction on Feline Transitions’ use of a contribution.

Use the legal name and EIN exactly as shown.
For an unrestricted charitable gift supporting the Lifetime Care Program, name Cats in the City Rescue, EIN 33-2724653, and include “Feline Transitions Lifetime Care Program” in the memo or purpose field. Mail notices or checks to the address shown. Request written acceptance before imposing a restriction or assigning any fiduciary or care role.

Role and restriction boundary
Do not assign duties by beneficiary form alone.
Do not name Feline Transitions as trustee, personal representative, attorney-in-fact, trust enforcer, owner, residential provider, or restricted-gift recipient without prior written acceptance. A beneficiary designation or gift does not itself create a care, custody, residency, or fiduciary obligation.
