Transition response

Tell the Team when a care plan may need to become action.

Use this report for an enrolled household after death, incapacity, hospitalization, displacement, caregiver failure, or another event that may require the plan.

If a person or cat is in immediate danger

Contact 911, local emergency services, animal control, or an emergency veterinary hospital first. Then notify Feline Transitions so the documented care response can begin.

    VerifyThe Team confirms the reporter, enrolled household, event, and current location of the cats.

    StabilizeImmediate welfare, access, medication, transport, and temporary-care needs are assessed.

    ActivateThe Team follows the accepted Path A or Path B agreement and records each action.
