Health and veterinary care

Record primary and emergency veterinarians, diagnoses, medications, allergies, pharmacy details, preventive care, mobility, dental needs, and monitoring schedules.

Daily life

Record food, feeding times, litter type and location, sleep, play, hiding places, social relationships, handling preferences, triggers, calming methods, and household rules.

Identification and transition

Include current photos, microchip details, physical identifiers, carrier location, transport needs, emergency contacts, and end-of-life preferences subject to veterinary judgment and applicable law.

Program boundary

Only accepted agreements create program duties.
